How to Complete an Assessment
We offer different assessments for schools, districts, and out-of-school time sites.
Schools
How do I find the Assessments?
Once you have logged in to the Action Center and joined an organization, you’ll be able to see the link for that site's Assessments.
On your Dashboard, each organization you have joined will have a link to your available Assessments.
Once you’re viewing an organization, you can access your available Assessments via a link in the left side bar or a link in the Overview box.
How do I start or update an Assessment?
You can click on any Topic to get started. There are no required topics, and no required questions. Questions can be completed in any order.
Once you select a Topic, you will see the list of criteria for that topic.
Next, click on the criteria to select your answer.
You will then see the question detail page where you will select the appropriate response. Be sure to click Save or Save & Next to move on to the next question. Check out the helpful links and information related to the question, located to the right of the question.
If you would like to print or download your Assessment, go to your Dashboard and click on View under the REPORT column.
Districts
Note: In order to complete a District Assessment, you must be a District Team Member. Click here for instructions on becoming a District Team Member.

Thriving Schools Integrated Assessment

Thriving Schools Integrated Assessment: District Edition
The Thriving Schools Integrated Assessment: District Edition is an evidence-based self-assessment tool to support districts in identifying strengths and opportunities for improvement regarding policies and practices that promote student achievement and the well-being of students, staff, and teachers. Healthy Schools Assessment
The Healthy Schools Program Assessment is a version of the School Health Index that addresses the health topics in the Healthy Schools Program Framework of Best Practices: Policy & Environment, Nutrition Services, Smart Snacks, Health & Physical Education, Physical Activity, and Employee Wellness. The CDC retains the full comprehensive School Health Index at www.cdc.gov/healthyYouth, addressing seven health topics: physical activity and physical education, nutrition, tobacco use prevention, alcohol and other drug use prevention, chronic health conditions (e.g., asthma and food allergies), unintentional injury and violence prevention (safety), and sexual health.
Healthy Districts Assessment
The Healthy Districts Assessment allows district leaders to provide a suggested response to their schools for some of the criteria on the Healthy Schools Assessment. For more information, please view our Healthy Districts Assessment Guide.
RISE Index
The RISE Index is a tool to help schools assess, prioritize, and plan key activities that promote the social-emotional health of staff and students. The RISE Index supports schools in increasing staff job satisfaction, including reducing staff stress, improving safety, connectedness, and relationships among students and staff, boosting student and staff social-emotional learning, and increasing the efficiency and effectiveness of mental health support systems. RISE Index: District Edition
The RISE Index: District Edition supports the well-being of district-level staff and enables districts to focus their efforts on supporting school-level social-emotional health policies and practices, in collaboration with other key stakeholders. Healthy Out-of-School Time Assessment
The Healthy Out-of-School Time (HOST) Assessment can help identify your site’s health priorities and highlight areas for improvement, using the National AfterSchool Association’s Healthy Eating And Physical Activity (HEPA) Standards. For more information, please view the Healthy Out-of-School Time Assessment Guide. There is also a HOST Assessment for overseeing organizations. This Assessment allows out-of-school time leaders the opportunity to provide suggested responses to their sites for some of the criteria on the HOST Assessment.

- You can work on your Assessments at your own pace, be sure to save any work before logging off.
- To view the latest on when your Assessments were updated, and by which team member, click on your Dashboard and then click on the links to your Assessments.
- Only team members can update Assessments for their organizations.
